PLEASE READ CAREFULLY BEFORE SIGNING

In consideration of being allowed to participate in activities at The Bunkerr Golf Lounge LLC (“The Bunkerr”), located in Plantation, FL, I acknowledge and agree to the following:

I understand that participation in golf simulator activities involves inherent risks, including but not limited to:

• Being struck by golf balls, golf clubs, or equipment.
• Slips, trips, or falls on or around simulator bays and lounge areas.
• Consumption of food and beverages, including alcoholic beverages.
• General risks associated with indoor sports and recreational activities.
   

I voluntarily assume all risks, whether known or unknown, related to participation at The Bunkerr.

I hereby release, waive, discharge, and covenant not to sue The Bunkerr Golf Lounge LLC, its owners, employees, contractors, or affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, accident, or loss (including property damage) that may occur during my participation.

In the event of injury, I authorize The Bunkerr staff to obtain medical treatment on my behalf and assume responsibility for all associated costs.

I acknowledge that The Bunkerr serves beer and wine for on-premise consumption. I agree to consume alcohol responsibly, comply with all applicable laws, and refrain from participating in simulator activities if impaired.

If the participant is under 18 years of age, this waiver must be signed by a parent or legal guardian, who agrees to the above terms on behalf of the minor.

I have read and understood this waiver, and I voluntarily agree to its terms. I understand that I am giving up legal rights by signing this document.
